Document Preparation Services
Pursuant to California's Business & Professions Code, sections 6401, et seq., a Legal Document Assistant is permitted to assist the general public in preparing documents at their direction. In order to facilitate the preparation of those documents, I have developed questionnaires/ worksheets for each of the subject areas for which I provide document preparation services. ESTATE PLAN
If you wish to create an estate plan, which would include a revocable trust, pour-over will, advance health directive, HIPAA privacy authorization, power of attorney for asset management, general authorization, and trust transfer deed, download the Estate Plan Worksheets.
The cost of a marital estate plan is $650.00; the cost a single-person estate plan is $450.00.

FAMILY LAW
If you wish to file to divorce, legal separation, or nullity of your marriage of domestic partnership, download the Family Law Worksheets.

The cost of an uncontested/default family law matter is $250.00. Additional work and modification of orders preparation is billed at $100.00 per hour. The court filing fee is $435.00 ($450.00 if filing in Riverside County).
SUMMARY DISSOLUTION
If you have been married five or less years and own no real property and have no children, download the Summary Dissolution Worksheets.
The cost of preparing a Summary Dissolution is $150.00; the court filing fee is $435.00 ($450.00 if filed in Riverside County).
PATERNITY
If you are not married but share a child and need custody, visitation, and/or support orders, download the Paternity Worksheets.
The cost of document preparation is $250.00; the cost filing fee is $435.00 ($450.00 if filed in Riverside County).
PROBATE
If you need to probate the estate of a loved one who has passed away, download the Probate Worksheets.
The cost of document preparation is $500.00; the court filing fee is $435.00 ($450.00 if filed in Riverside County). Additional filing fees will be incurred during the proceeding, as well as a publication fee and probate referee fee.
CONSERVATORSHIP/GUARDIANSHIP
If you need to obtain a conservatorship over an adult or a guardianship over a minor, download the Conservatorship or Guardianship Worksheets.
The cost of document preparation is $250.00; the court filing fee is $435.00 ($450.00 if filing in Riverside County). If additional document preparation is needed, such work is billed at $100.00 per hour.
BUSINESS FORMATION
If you wish to create a corporation, a limited liability company, or a professional corporation, download the Business Formation Worksheets.
The cost of document preparation is $500.00, which includes the preparation of your articles of incorporation/articles of organization, application fee, preparation of by-laws, sample minutes, sample letters, sample resolutions, and preparation and filing of the initial Statement of Information.
